You can not fail the accreditation for making a mistake. The accreditation authority can fail one or more of the answers you provided in the self-assessment. However, you can always amend your product and provide a new answer to continue with the accreditation. You can even use the “Comments” option to ask for clarification as to why your answer was failed. 

The entire process has no limitation regarding how many times it can be repeated.
